Our Policy
REFUND POLICY:
NO CASH REFUNDS. (It means: no refunds on credit cards or checks. If by any personal reason a customer decides to withdraw from our services, a credit will be issued to the family in question with NO expiration date).
MAKE UP CLASS POLICY:
A make up class will be scheduled only in case of illness or injury with written verification from a physician. NO MAKE UP CLASSES ON SATURDAYS. A credit can be issued ONLY if there is no other class available for make up (example: the class only runs in ONE day of the week, or if the participant is in our Summer clinics).
BIRTHDAY PARTY POLICY:
Birthday party payment should be received IN FULL in order to be processed.
You can reserve a date and time online for 48 hour without payment. If you fail to pay within the 48 time period your reservation for the date and time will be cancelled
The calendar does reflect the current times and dates available. If you would like to check on other possible dates please contact the office.
Please be sure of the date and time, no last minute changes or cancellations allowed.
If by any personal reason a customer decides to withdraw from having a Birthday party two weeks before the date, a CREDIT IN FULL will be issued to the family in question, with no expiration date.
*Sorry, no confetti or pinatas are allowed at our parties.
FIELD RENTAL POLICY:
All team managers/person-in-charge must fill out a form containing contact information, such as: phone number (home and/or cell/work) and address.
All players must sign a release of liability form before going on the field.
Payment for rental is requested at the time of booking.
When paying by cash, always request a receipt (it should contain at least the date and time of rental).
All checks should be payable to United Indoor Soccer.
For long term rental:
United Indoor Soccer requests for rentals due in advance.
A deposit is requested at the time of booking the reservations.
The deposit will not be lost if cancelled at least two weeks prior to the cancellation date.
Any deliquency in payment will result in losing the time requested.
GENERAL RULES OF THE FACILITY
Individuals utilizing this facility do so at their own risk. The property owner(s), and staff of United Indoor Soccer (UIS) assume no liability for any injuries or accidents that may occur. Please reference Liability/Injury Waiver, Release Forms and signage posted within the facility.
Conduct within the facility should be in the spirit of good sportsmanship as is on the field. Please NO ALCOHOL OR DRUG USE prior to playing.
Should you find or lose any items, please report this immediately to the UIS staff.
NO ALCOHOL, SMOKING, GLASS CONTAINERS, OR GUM ALLOWED IN THE FACILITY.
NO FOOD, DRINKS, SPITTING, OR ABUSIVE LANGUAGE ALLOWED ON THE FIELD.
We reserve the right to refuse play or service to anyone.
Any orders will be CANCELLED, if the payment is not received within 48 hours of submitting a form online.
